The Canadian Foundation for Healthcare Improvement (CFHI) Evaluation Committee met in-person today to discuss program goals and the evaluation plan for the Bridge to Home Collaborative program. The purpose of the Collaborative is to support teams across Canada improve the patient experience and the quality of care during transitions from hospital to home.
